Everyone matters at a CHC - we have something for everyone!: Our Bridges CHC team includes doctors, nurse practitioners, nurses, dietitians, diabetes educators, a mental health nurse, a social worker, health promoters and a community health workers and more. To promote healthy living in our communities, our programs and peer support groups can help you with smoking cessation, diabetes education, mental health, chronic pain management, stress management, exercise and healthy eating. If you do not have a doctor, our dedicated team of professionals can meet your everyday health care needs.
